A nurse walked out of an operating theatre and into the Maasai wilderness to understand why women were dying before they could be saved. This is what he found — and what he built.

In 2017, serving as Head of Department in the Operating Theatre at a hospital in Kajiado, a pattern began to emerge. Women — mostly of Maasai origin — arrived in critical condition after four to five days in labour. Obstructed labour. Sepsis. Haemorrhage. Fetal demise. Nearly 90% of these emergency cases carried poor prognoses.

One morning, the moment arrived that would change everything. A young woman admitted following a 28-week miscarriage complicated by retained placenta. Her husband — a Member of Parliament — stood in the corridor to pray. A national leader, made wholly vulnerable by a preventable complication.
If someone with influence and resources could still encounter such delays — what chance did the ordinary woman in a remote village have?
Repetition transforms tragedy into pattern — and pattern demands explanation. The same stories repeated: women arriving exhausted after decisions delayed by distance, cost, trust, and cultural dynamics. Each delay was a rational adaptation to a system that did not serve them.
Inside the hospital there was structure — skilled personnel, equipment, protocols. Outside: long distances, limited facilities, informal providers, cultural complexity. And between these two realities, a wide, dangerous gap. Most suffering occurred in the space between illness and access.
Located approximately 90 kilometres from Nairobi's central business district, deep within the semi-arid landscape of Kajiado County, Kamukuru sits in a valley surrounded by scattered settlements — Losinyai, Leremit, Nkaama, Koora, Naserian, Kamukuru Oiti, and more. To reach it is to transition into an entirely different health reality.

The nearest formal health facility was a Level 2 dispensary, approximately 2 kilometres away — but operationally limited and not designed for comprehensive maternal emergencies. The next available facility was nearly 10 kilometres further. People do not live next to services here. They live between them.
After weeks in the field, the question changed. Not What is happening here? but What am I going to do about it?
The answer was presence. Sustained, committed, community-rooted presence. A modest structure was rented from Mr. Rakua in Kamukuru. Trust was built through Celestine and community elders. Patients began arriving — not because the system was strong, but because someone had finally shown up.
From that fragile beginning, NOSIM Medical Centre grew into what it is today: a trusted healthcare provider with an expanding presence across Kajiado County, driven by compassion, resilience, and a commitment to the underserved.
